‘9-1-1’ Season 10 Episode 4 Recap & Ending Explained: Buck’s Theo Crisis and the Fake Doctor Case Explained
After the volcanic chaos of the Lava Land storyline, ‘9-1-1′ Season 10 Episode 4 trades spectacle for something quieter and far more personal. ABC aired ‘Imposter Syndrome’ on Thursday, Nov. 5, and most of its drama unfolds inside the 118 firehouse and Buck’s chaotic home. It is an hour about doubt, pride and what happens when people refuse to admit they were wrong.
That earlier disaster wrapped with Maddie diverting the lava flow into a subway and Athena being rescued just in time. It cleared space for the series to return to the fallout from the Season 9 finale, ‘Hearts and Flowers,’ which ended with Buck fostering Theo, the child he helped his friends conceive as a sperm donor. Since then, Buck’s emergency room visits have piled up so much that hospital staff began wondering whether he was a victim of domestic abuse.
A Cold War Inside the 118
The central tension begins with Buck and Eddie still not speaking after their argument over Theo. TV Insider reported that Hen, Chimney, Ravi and Harry treat their awkward kitchen exchange like live entertainment, and the cutting remarks continue in the engine between calls. Ravi, meanwhile, is caught in the middle.
Nobody who followed the build-up should be surprised. Stark told TVLine in October that the conflict was “going to last longer than I expected it to.” He suggested the two friends would remain at odds for a while.
At home, four-year-old Theo keeps wrecking Buck’s house, and TV Insider noted that the therapist, Dr. Murphy, recommends a referral to a therapist for adults. Stark has been sympathetic toward the boy from the start. In an interview with TV Insider, he said, “he is a handful. I think justifiably so.”
The Park Arrest That Changes Everything
The turning point arrives at a park, where onlookers see Buck dragging a kicking, screaming Theo to his car. Buck is arrested on a false kidnapping accusation after Theo tells a police officer that Buck is not his father. The humiliating moment forces Buck to ask whether Theo will ever see him as more than the stranger who took him from his parents.
He gets his answer when he notices Theo reenacting the fatal crash that killed his parents with toys. According to TVLine, Theo seems unhappy when Buck tries to add a toy firetruck to the scene.
‘Imposter Syndrome’ Ending Explained
Before the credits roll, Buck and Eddie finally end their cold war. Buck apologizes for lashing out, Eddie admits he overstepped, and Buck concedes his friend was right.
The episode still closes on a sad note, because Buck concludes he is not fit to be Theo’s father. TVLine noted it is unclear whether he will follow through, so the decision may be a low point rather than a final answer. SoapCentral similarly suggested the arrangement between Buck and Theo will change in upcoming episodes.
Fangirlish argued that Buck is not solely to blame, since Theo has had little structure or emotional grounding. The outlet’s reviewer also hopes Buck will listen to Eddie, resist self-doubt and start therapy.

Athena, May and the Fake Doctor
Elsewhere, Detective Athena Grant hunts a man who posed as a doctor at a car accident scene and gave terrible medical advice to vulnerable victims. TVLine reported that he comes from a family of doctors and that a neurological disorder has convinced him he is one too.
His deception reaches May’s hospital, where he pressures her into assisting with an unnecessary appendectomy. May, who was recently told she is too nice, figures out what he is doing and sedates him.
Maddie is fooled by the same man over the phone, and after an intense car crash call she considers quitting. She even floats a part-time job to Chimney, but a woman she saved from a carbon monoxide leak thanks her at the call center and reassures her. The man is eventually caught and receives medical help.

Where Season 10 Goes From Here
Fan reaction has been mixed. Some TVLine readers say the Buck and Theo storyline needs to go away, while others like the idea of Buck raising his son but want less chaos.
With the Lava Land crisis behind the 118, the real question is whether Buck’s decision is a plan or a moment of panic. Given how quickly he and Eddie repaired their friendship, Buck at least has his best friend back in his corner.
Should Buck step back from raising Theo?
