Very few shows get to be the longest-running anything, yet ‘Grey’s Anatomy‘ has quietly held that title among primetime medical dramas for years. After 23 seasons, every new premiere comes with the same lingering question about how much longer Grey Sloan Memorial can keep its doors open. The latest interview with the showrunner offers fresh answers about the future, a very special guest-star storyline and a spinoff on the horizon.

The season opener, “Collide,” picked up a month after the previous finale, marking the show’s first time jump between seasons in 4 years. It also introduced James Wolk as Dr. Seth Norton, the new chief of surgery, following the exits of Kevin McKidd’s Owen Hunt and Kim Raver’s Teddy Altman. Along the way it dropped a pregnancy twist for Simone and a resignation for Jo.

Now showrunner Meg Marinis has weighed in on what comes next. In an interview with Variety, Marinis addressed whether this could be the final season, saying “No one’s told me that the end is coming.” She added that she is grateful for that, and the show’s episode count has not been confirmed to her yet.

Marinis acknowledged that every year could be the last for a show this old, but for now the network is still letting the team make episodes. Reports have pointed to budget cuts, including a shorter run than the usual order and the exits of 2 long-running cast members. Even so, ABC has not announced an end date, and Marinis is simply focused on telling stories while she can.

That tone has been consistent for years. Marinis has said previously that she hopes to stay as long as the network will have her. For viewers, the lack of a farewell announcement is reassuring, even if the show’s long-term future is still unwritten.

Marinis explained that she wanted to explore the upstairs-downstairs side of medicine, a topic she described as increasingly relevant as healthcare disparities grow. Rather than bring back a familiar face to lead the hospital, she opted for a corporate outsider. Norton’s appointment reflects the financial sacrifices modern hospitals make just to stay open to the general public.

The VIP wing, known as the Olympic Floor, also sets up a recurring conflict for the doctors. Meredith clashes with Catherine and Richard after a VIP patient delays care for someone else. It is a classic ‘Grey’s Anatomy’ dilemma, pitting the hospital’s oath of care against its bottom line.

The conversation also touched on a storyline that has fans curious. The third episode of the season features a Dolly Parton impersonator as a patient, a story pitched by that episode’s writer, Jase Miles-Perez. Marinis said the writers thought it was hilarious and endearing.

The episode has since taken on extra emotional weight. Filming was reportedly underway when Parton died in August at age 80, and Debbie Allen directed that storyline. Marinis said it was emotional for the team, especially for Allen. What began as a playful idea has become a heartfelt tribute to the country legend.

Marinis also gave an update on the West Texas spinoff she is co-creating with Shonda Rhimes. She said it is in the works, set in rural West Texas, and will connect to ‘Grey’s Anatomy.’ ABC gave the series a straight-to-series order for midseason 2027, and Ellen Pompeo is an executive producer.

The new show is described as an edgy drama about a team at a rural medical center, the last chance for care before miles of nowhere. It will feature an entirely new team of doctors and explore medical deserts in isolated communities. According to Marinis in a separate interview, characters from the spinoff will appear on ‘Grey’s Anatomy’ this season, though the episode has not been locked.

Taken together, the interview paints a picture of a show that is evolving rather than winding down. Marinis is juggling 2 shows, the hospital is redefining itself and the cast is changing. Pompeo’s Meredith also continues to appear in a reduced role.

For longtime viewers, there is plenty to look forward to, from the Parton tribute to the new Texas crossover. The series also streams the next day on Hulu, keeping it among the most-watched shows on television. If this is the beginning of the end, it is doing a remarkable job of staying fresh.

Is it time for Grey's Anatomy to end, or should it keep going?
