Halloween has a way of bringing out the strangest tensions inside a hospital, and ‘Chicago Med‘ leaned into that spirit this week. Between a heartbreaking delivery, a family crisis and a boardroom conspiracy, the Halloween episode ending left Gaffney Chicago Medical Center with more questions than treats.

The episode, “Tomorrow, and Tomorrow, and Tomorrow,” aired on NBC on October 28 at 8 p.m. ET, with Peacock streaming the following day. It is the fourth hour of a season that kicked off on October 7 with “Shots Fired.”

Sharon Goodwin is no longer running things. She returned to Gaffney as a nurse after Miranda asked her to resign from her previous position. The season premiere closed with Nurse Doris appearing to confess on Goodwin’s doorstep that she was responsible for the leak at the center of the HIPAA scandal.

This week, Goodwin and Charles are on the same side after clearing the air, and the plan is to take Miranda on together. The episode opens with Goodwin waiting in a parking garage to get secret information from Peter, another board member. She still doesn’t know what Miranda is up to.

The emotional center of the hour belongs to Asher. She and Kai-Tanaka treat a pregnant woman who fell, and Asher rushes her into labor. The baby is delivered by C-section, but the mother begins to hemorrhage, and the OR turns chaotic as the team fights to save both lives.

The baby is pronounced dead after 20 minutes of resuscitation efforts. Asher breaks the news to the parents, and Kai-Tanaka offers them the chance to hold their daughter. Afterward, Archer finds Asher on the roof and tells her about losing a baby patient after his son was born, assuring her she did nothing wrong by showing emotion. Her patient later wakes with temporary amnesia and doesn’t remember what happened.

Archer treats Dr. Abrams’ ex-wife, Cynthia, who was brought in by their daughter, Lucy. Cynthia has a kidney infection from her catheter, and Archer wants her kept for observation. Charles notices certain behaviors in Lucy and tells Abrams that she may need more help, but Abrams takes offense at the suggestion.

The situation comes to a head when Abrams finds Lucy exhibiting those behaviors and she has a panic attack. Charles talks her through it, and he later says he believes she has OCD and recommends a treatment facility. Lucy makes her father promise to take care of her mother while she takes care of herself.

Frost is on the fence about applying for chief resident until Wu pitches the downsides, including paperwork and administration. While working a case with a young patient named Caleb, they discover that his brown teeth are tied to a genetic disease that is now treatable. Frost then learns Wu manipulated him into dropping out, and he decides to submit his application to Dr. Hart.

One reviewer wrote that Wu was worse than ever, noting she took credit for Frost’s diagnosis of a rare disease before asking for the job herself. The result is a direct rivalry between the two residents.

Ripley and Goodwin also treat Nara, a patient preparing for a transplant and a trip to Italy for her 10th anniversary. Ripley calls her oncologist, Dennis Washington, who is Goodwin’s ex. A biopsy reveals she has a different form of cancer than first diagnosed, and this form can be cured.

Dennis is shocked to see Goodwin working as a nurse. He apologizes for how their relationship ended and admits he still loves her, but Goodwin doesn’t appear interested in getting back together. She also advises Nara’s wife, who was planning to leave before the diagnosis, that she doesn’t have to decide anything right away.

After Goodwin sees Dennis off, Peter returns with more of Miranda’s secrets. He tells her Miranda was never advising the board to search for Gaffney’s next CEO. She wants the job for herself.

That reveal reframes the season’s power struggle. Goodwin’s removal and the instability at the hospital now look like steps toward one goal, which gives Goodwin and Charles a clearer target. It also makes the earlier hints that Miranda was reshaping Gaffney feel far more deliberate.

The episode ends on a softer note. When Asher gets home, dozens of costumes for Mabel are laid out, and she kisses Archer, falling into an embrace after a very hard day.
