Grey Sloan Memorial is a very different place in ‘Grey’s Anatomy‘ Season 23, where the first time jump between seasons in four years and a gleaming new VIP floor have reshuffled the hospital’s priorities. Episode 4, titled ‘Love on the Brain,’ aired Thursday, Nov. 5, at 10 p.m. ET on ABC. It leans on romance, family drama and some uncomfortable questions about who gets the best care.

The season opened with ‘Collide,’ which introduced James Wolk’s Seth Norton as the new chief of surgery and ended with Jo telling Link she had quit her job. Kwan, meanwhile, was still fired after secretly returning to save the life of his online French teacher. Those threads shape nearly everything in the fourth hour.

Bailey, Kwan and the VIP Floor

At the center of the episode is a VIP patient with a long-undiagnosed illness. Bailey and Kwan finally solve the case, and Bailey starts to wonder whether the VIP floor could help all patients. TV Fanatic noted that she pushes harder after learning the patient’s children pooled their savings to get answers.

Webber, meanwhile, wants Norton gone for dismantling his program, while Bailey plans to have dinner with the new chief. Kwan’s work earns praise from Webber and permission to scrub in on Bailey’s surgery.

Family Drama and Secrets

Lucas notices Simone is struggling and asks if she is okay. She admits she “went through a thing” but fears that talking about it could keep her from returning to work, then invites Lucas to move into her house. She also says her boyfriend Wes will understand.

The paternity of Simone’s baby is still unresolved, and showrunner Meg Marinis stressed in an interview with Deadline that “It does not automatically mean it is Lucas.” SheKnows has noted that even Simone does not know whether Wes or Lucas is the father.

Elsewhere, Toni cancels Amelia’s surgeries to surprise her with a weekend getaway. Amelia dislikes surprises and says planning ahead helps her stay sober, but they make up, and she still wants to go.

The quietest storyline belongs to Jo, who attends a birth trauma support group. She bonds with another struggling mother, admits that the sound of a hospital monitor haunts her, and exchanges numbers with her new friend.

Jules and Winston Cross the Line Again

After a patient dies on the table, a heartbroken Jules confronts Winston, her ex and her boss. She tells him his efforts to help her are confusing, and she argues that if they want distance, they need to commit to it.

Winston says he would rather commit to Jules instead, and the two share a kiss in his office. TV Fanatic’s reviewer thought it was about time Jules developed better ways to cope with losses.

‘Love on the Brain’ Ending Explained

The hour closes on Kwan. Just as he regains Webber’s approval and leaves the hospital for the night, a well-dressed stranger approaches him. Kwan recognizes the man at once and says, “…Dad?”

TVLine reported that the father is played by Russell Wong. Kwan previously helped pay his late mother’s hospital bills during her terminal illness, and he does not look thrilled to see his father, which suggests a difficult relationship.

The twist lands on a character whose future was a mystery even to his portrayer. Harry Shum Jr. told E! News in May that he was clueless about where Kwan was headed, saying, “I wish I knew,” as SheKnows reported.

What Comes Next

The scene suggests ‘Grey’s Anatomy’ is finally ready to explore Kwan’s life outside the hospital. Jo, by contrast, leaves Grey Sloan with Link, smiling, which is a rare happy note for her.

Between a father’s reappearance, a rekindled office romance and a VIP floor that Webber and Bailey view very differently, Season 23 has plenty to resolve. Marinis has also said the premiere’s fallout, including Jo’s resignation, will reach well beyond a single episode, according to The Eastern Herald.

SheKnows treated Simone’s confidence that Wes will understand skeptically and framed the situation as a returning love triangle. Wes also gets a rough day, covering for Warren several times only to be left out of a surgery.

Are Jules and Winston making the right decision by giving their relationship another chance?
