Rosie O’Donnell Reveals Dramatic Facelift Transformation With Before-and-After Photos: “Best Possible Outcome”
Rosie O’Donnell has opened up about her recent facelift and shared before-and-after photos that show the subtle changes from her procedure. The information comes from her Substack essay and Instagram post published earlier this week, where she explained both the emotional and physical sides of her decision.
The 64-year-old actor and comedian revealed that she had a lower deep plane facelift in January. She described the procedure as one that lifts deeper tissue under the facial muscles rather than tightening the skin too much, which can sometimes create an unnatural look.
She posted the photos with a simple caption, saying, “THE B4 & AFTER — story on substack.”
In her essay, O’Donnell explained that she never expected to have cosmetic surgery. She said she used to strongly believe that facelifts were something she would never consider. She admitted she even saw it as a moral issue connected to aging and feminism.
She wrote, “I used to feel very strongly about facelifts. Not casually — morally. I had assigned myself as head of all women who would never — ever.”
Her perspective began to shift after she lost around 50 pounds while taking the prescription weight-loss medication Mounjaro. She said the weight loss changed her appearance in ways she was not prepared for, especially because it led to more visible wrinkles and a tired look in her face.
O’Donnell explained that she initially tried to accept the changes in a positive way. She told herself that aging naturally was something to embrace. But she also admitted she began questioning that mindset. She wrote that she found herself thinking, “Umm how earned does it have to look?”
She also shared that her child, Clay, strongly opposed the idea of surgery. Clay reminded her that many young people look up to her and would be influenced by her choices. Clay reportedly told her, “Young women look up to you,” and added, “I wouldn’t be able to respect you if you did it.”
Those comments made O’Donnell reconsider the procedure for a while. She said it forced her to think about her own beliefs and whether she was being inconsistent with the values she once held strongly.
After time and reflection, she decided that she needed to make a choice based on what felt right for her personally. She found a surgeon she trusted, someone recommended by friends who still looked natural after their own procedures.
Following the surgery, O’Donnell described the result as subtle and natural. She said she now looks like a more rested version of herself rather than a different person entirely. She also joked that, surprisingly, no one around her seemed to notice the change.
She wrote, “Not one person in my life has noticed. I went through a full existential feminist crisis, had my face and neck surgically altered, and the result is… zippo.”
Despite the lack of reaction from others, she said the experience brought her peace. She explained that she no longer feels like she is constantly debating her reflection. She wrote, “I didn’t disappear, I didn’t become someone else — I just stopped arguing with the mirror. And maybe that’s enough.”
O’Donnell’s reflection highlights how personal and complex decisions about appearance can be, especially when mixed with public expectations, aging, and identity.
