‘Iron Lung’ Ending Explained: What Simon’s Final Sacrifice Really Means

Markiplier’s self-made sci-fi horror film has done the unthinkable at the box office, but it’s the haunting, unresolved final moments of ‘Iron Lung’ that audiences can’t stop talking about. For a movie built around one man alone in a sealed submarine navigating an ocean of human blood, it somehow manages to deliver one of the most emotionally complex endings in recent genre cinema.

What makes the climax so divisive is that it refuses to offer easy comfort. Simon doesn’t make it out alive, and the film deliberately leaves the door open on whether his sacrifice even matters. That ambiguity is the whole point.

What Actually Happens When the ‘Iron Lung’ Goes Dark

Simon, the convict at the center of the story, is forced into piloting the SM-13 submarine through an ocean of literal human blood on the desolate moon AT-5. As the mission deepens, he encounters the wreckage of the SM-8, a submarine built for extensive surveillance and originally intended for scientists rather than convicts.

He recovers the wreck’s black box and integrates its data into his onboard recorder, uncovering that the Consolidation of Iron was fully aware of the dangers lurking in the blood ocean and continued deploying prisoners regardless.

Ava, the mysterious voice on the intercom, eventually apologizes that she cannot pull Simon out. From that moment forward, his fate is sealed. A hivemind creature then destroys Ava’s vessel entirely, severing the last direct line of rescue and leaving Simon completely alone in the deep.

Simon makes his final choice: he attaches the black box containing the SM-8 data to a life vest buoy, then sabotages the Iron Lung’s hull so it will implode under deep-sea pressure, destroying both the submarine and himself. Both Simon and the creature perish in the implosion, with the focus shifting to the life vest and the intact black box, implying that while Simon’s life ends, the data and the hope it represents may endure.

The Blood Ocean and the Quiet Rapture Connection

The world of ‘Iron Lung’ is shaped by an event called the Quiet Rapture, which caused all stars and planets to mysteriously vanish, leaving only those on space stations and spaceships to survive with humanity reduced to very few. The blood ocean on AT-5 is not simply an alien curiosity. It sits at the intersection of that catastrophe and something far older.

During his hallucinatory episodes, Simon converses with a woman who speaks obsessively about a mysterious light beneath the ocean, and these visions imply a direct connection between the blood ocean and the Quiet Rapture itself.

The black box logs from the SM-8 reveal that the blood ocean is the habitat of a cosmic entity, and that the Quiet Rapture was not an accident but something closer to a harvest.

The blood ocean ties into the broader lore as a likely byproduct of the Quiet Rapture, a cataclysm that devoured worlds, with failed missions adding to the sea of lost lives. This is cosmic horror at its most uncomfortable, where the universe is not indifferent but actively malevolent, and human curiosity only pulls survivors closer to whatever consumed everything else.

Simon’s Sacrifice and What the Life Vest Symbolizes

The final image of ‘Iron Lung’ is deliberately stripped of triumph. The last visual is the life vest floating on the surface of the blood ocean, its beacon blinking, suggesting that the data might eventually be found even though Simon has paid the ultimate price.

Simon refuses to let the mission be a total failure, but he also refuses to die passively. Rather than fleeing or waiting for an end that will come regardless, he overloads the sub and drives it directly into the creature. Opposite his grim ending is the gift he has left for humanity before he dies, which is hope and a chance to possibly survive. It is a distinction the film insists on drawing, even as it denies any cathartic release.

The movie does not offer a happy ending in a conventional sense. Instead, it emphasizes sacrifice, uncertainty, and the indifference of the universe, which are the trademarks of cosmic horror that distinguish ‘Iron Lung’ from more straightforward sci-fi narratives.

Markiplier’s Directorial Vision and the Film’s Unexpected Success

The film’s creative DNA is inseparable from its maker. Development on an ‘Iron Lung’ film adaptation began in April 2023, when Markiplier announced he would be adapting the game into film, self-writing it, self-financing it, and starring in it himself. Markiplier had first played the short indie horror game in summer 2022 and was so taken by its claustrophobic atmosphere that he decided to turn it into his first directorial effort made for theaters.

While still a relatively inexperienced actor in traditionally produced movies and television, he carries all 127 minutes of the film almost entirely alone, and to his credit, Markiplier delivers the intensity needed in the final act when Simon makes a desperate and impressively gruesome sacrifice. The film was produced on a budget of $3 million and features a cast that includes Troy Baker, Elsie Lovelock, Caroline Kaplan, and Seán McLoughlin.

How ‘Iron Lung’ Rewrote the Rules for Independent Horror

What followed the release was an industry story as unlikely as anything in the film itself. ‘Iron Lung’ pulled in $18.19 million at the domestic box office on opening weekend alone, more than six times its reported $3 million budget. The indie horror film has since generated over $51 million worldwide, earning a critics score of 62% and an audience score of 87% on Rotten Tomatoes.

Speaking with BBC Newsbeat, Markiplier called the movie’s success a win for YouTubers, expressing hope that such major successes would be normalized over time. The box office run has placed ‘Iron Lung’ in the top 40 of the highest-grossing independent films of all time, ranking alongside A24’s ‘Midsommar’ and 2014’s ‘Nightcrawler.’

For a film distributed without a major studio, released by a YouTuber on his own terms, and built around a man trapped in a tin box surrounded by blood, that is a cultural moment that is genuinely hard to overstate.

The ending of ‘Iron Lung’ does not give viewers the release they may want, and that is precisely why it lingers. Simon is gone. The beacon blinks. The question the film refuses to answer is whether anyone will ever come to retrieve it, and whether humanity deserves the hope he died to preserve. What do you think the blinking beacon on that life vest really means for the surviving remnants of humanity, and did Simon’s choice feel like heroism or futility to you?
