‘Scrubs’ Season 2 Episode 5 Recap and Ending Explained: A Birthday Dinner Leaves J.D. and Elliot Single and Sean Has the Last Laugh
‘Scrubs‘ has never been shy about turning ordinary moments into emotional detours, and the latest episode of the ABC revival is a perfect example. Titled “My Best Friend’s Birthday,” it aired on October 21 on ABC, with streaming on Hulu arriving the next morning.
The revival, which brought back J.D., Turk and Elliot more than 15 years after the original finale, has spent its second season continuing to explore how the trio’s lives have changed. J.D. and Elliot are divorced and share a son, but their love lives have remained tangled, with J.D. dating Rachel Bilson’s Charlie and Elliot back with her old flame Sean, played by Scott Foley.
That setup pays off in a big way here. By the end of the episode, J.D. and Elliot are both single again after catching Charlie and Sean making out in the hospital, an unexpected twist that also gives Sean a satisfying last laugh against his longtime rival.
The trouble begins when J.D. and Elliot both struggle to find time for their partners. J.D.’s first day off in months is cut short when he’s called back to Sacred Heart, ruining a planned beach day with Charlie. Elliot, meanwhile, is stuck treating a young patient who somehow swallowed his entire state quarters collection while Sean waits around.
To make it up to them, the pair invite Charlie and Sean to Turk’s birthday dinner. Carla has taken the kids out of town, leaving Turk free to celebrate with a fancy steak dinner and his two best friends. But the hospital never lets anyone relax, and the newbies page Turk about a coding patient while J.D. and Elliot notice that their waiter’s eye looks like it is popping out of his head.

With the doctors pulled away, Charlie and Sean tag along to Sacred Heart, where they split an expensive bottle of wine while watching ‘Frozen’ in the pediatric unit. Meanwhile, J.D. and Elliot are reminded of how well they still work together, and they reflect on how hard life was right after their divorce. Elliot recalls bingeing Pringles from a can, while J.D. admits he slept at the foot of Turk and Carla’s bed like a dog.
They conclude that they are both in a better place and are happy to see each other moving on. J.D. even tells Elliot that he can tell Sean really loves her. It is a heartfelt moment, and also the setup for the punchline.

Just as the two finish congratulating each other, they catch Charlie and Sean kissing, with Charlie straddling Sean. Turk arrives in time to join his stunned friends and delivers a one-word reaction, “Damn!”
Later, at Lloyd’s Tavern, Charlie and Sean apologize for how things went down. The scene ends with Sean getting the last laugh, shooting down J.D.’s insistence that he is technically Elliot’s ex-ex-ex-boyfriend with a blunt “Nobody cares, J.D.!” And just like that, both leads are back on the market.
The twist does more than shake up the love lives. By removing Charlie and Sean from the picture, the episode pushes J.D. and Elliot back toward each other, and their easy teamwork at the hospital points to a possible reconciliation down the line. It’s the kind of slow-burn tease that the series has used for years.
It also gives Sean a satisfying turn. After years of J.D. coming between him and Elliot, Sean finally turns the tables, and he barely has to try. The episode does leave one piece of history unmentioned, since Sean also dated the mother of J.D.’s son back in the original run, something the revival has chosen not to revisit.
This episode follows a story about J.D.’s workload. In the previous installment, he faced a hard decision about whether to report a mistake that Turk made, which would end up costing the hospital, and he decided to cut his hours, learn to delegate and see a therapist. That context helps explain why J.D. is so desperate to make time for Charlie in this episode.
With both J.D. and Elliot single, the show has plenty of room to play. Fans will be watching to see whether the pair lean into the reconciliation the episode hints at, or whether the show keeps them circling each other a little longer. New episodes air Wednesdays on ABC at 8 p.m. ET, followed by a Hulu release the next morning.
What do you think should happen next for J.D. and Elliot?
